ACTIVATE has demonstrated real world outcomes: An average of 3.5 point reduction in hemoglobin A1c, 20 point reduction in systolic blood pressure, and 5 point reduction in diastolic.

Our company specializes in the design, development, and operationalization of equity-first digital-oriented solutions that support people and institutions to achieve their goals related to health and care.   We focus entirely on the safety net including community health centers, social care organizations, health center controlled networks, and rural health systems.  We promote and leverage the potential of technology-driven resources and  also acknowledge the critical role of human-driven processes in order to develop solutions that truly support at-risk and marginalized populations and the people and institutions that are involved in their care.

Health Tequity has extensive experience in the design, development, implementation, and evaluation of a wide variety of training programs for the workforce. We have created award-winning programs and models to help orient organizations and individuals to better address the digital divide in their communities, and experience supporting the workforce in this space for more than a decade. We have been able to refine our training models to reflect the exciting new developments in technology to meet the needs of a heterogeneous and ever-shifting workforce.
