TeamLET™
TeamLET is a model for improving primary care teamlets (providers and staff) communication, problem solving, and clinic efficiency. The program includes an 8-hour training for medical/dental assistants and other support staff, followed by a 4-hour joint training of providers and assistants.
The Health Coaching Accelerator Program assists clinics in developing and implementing health coaching within a primary care team model. This program focuses on training medical assistants, health educators, community health workers to provide health coaching for patients with hypertension and diabetes.
Health Coaching Model Clinic™
Health Coaching Model ClinicTM is a simulation-based training program for leadership development and innovation dissemination for the Health Coaching Accelerator Program. We offer an integrated approach aimed at successful implementation of health coaching in community clinics.
Community Health Worker/Digital Health Navigator Introductory Training Program
This comprehensive 10-hour training equips individuals with the necessary tools to enhance outcomes for medically underserved individuals and those at higher risk of adverse health outcomes.
